Volunteer Waiver/Parental Consent Form
A volunteer waiver is required from each volunteer prior to participating in a Keep Pennsylvania Beautiful adoption-related event. The parental consent portion of the form is required for each youth younger than 18 desiring to volunteer in an adoption cleanup.

Adoption Cleanup Reporting Form
Form for reporting results of adoption cleanup - to be sent within 2 weeks of event.

Recognition Sign Order Form
Form for ordering signs recognizing the adoption/cleanup effort.
